IGHBA values your feedback on the 2018 season. Below you will find the player feedback forms to print off, along with the link to the parent feedback form. Both forms should take no longer than 5-10 minutes to complete. We strongly encourage every player and parent to complete their respective forms. Your feedback is VERY valuable to help the organization improve.

For Players:
We ask that each player print off their respective form and complete to the best of their ability. Please have the player be as detailed as possible when giving feedback. When complete, either return to your coach/manager in a sealed envelope or email your completed form to Jesse Krebs at vicepresident@ighba.com.

For Parents:
We ask at least one parent/guardian for each player to complete the online survey for the 2018 season. If you have multiple players on multiple teams, you may complete one survey for each player. No more than two responses per player per team. Click the link below to be redirected to the survey.
